Option #1: $115.50 Certification fee 

Complete DEMOGRAPHIC FORM and email to CannaNurseHealing@gmail.com

Email to schedule a certification appointment. 

Before your appointment:

1. Complete Medical Cannabis application online by clicking here: https://medmj.ehawaii.gov/medmj/welcome

2. Fill out application information and upload your ID (driver's license, state ID, or passport). Designate Karen L Nishimoto, APRN as your certifying provider.

3. Pay State Application fee of $38.50 or 

Option #2: $154.00 (Certification fee + State Fee)
