Before your appointment:
1. Complete Medical Cannabis application online by clicking here: https://medmj.ehawaii.gov/medmj/welcome
2. Fill out application information and upload your ID (driver's license, state ID, or passport). Designate Karen L Nishimoto, APRN as your certifying provider.
3. Pay State Application fee of $38.50 or
Option #2: $154.00 (Certification fee + State Fee)

Renewal Certification (2-Year)

1. Complete Medical Cannabis renewal application online by clicking here: https://medmj.ehawaii.gov/medmj/welcome
2. Pay online State Application fee of $77.00 or
Option #2: $192.50 (Certification fee + State fee)
If we created your online account and initial application.
Then we do everything else for you! Just call to schedule a phone appointment.
Additional Requirements:
1. Medical record(s) confirming diagnosis.
2. Must have one of the following Identification:
- State Identification (any state)
- Driver's License (any state)
- Active & Valid Passport
Important Information
1. We are unable to certify patients who are pregnant or breastfeeding.
2. Hawaii Department of Health Bulletin: Cannabis and Your Baby
STATE OF HAWAII - MEDICAL CANNABIS PROGRAM RULES AND THE LAW
- One caregiver per patient; one patient per caregiver.
- One grow site per registered patient. - No more than 10 cannabis plants, jointly between a registered patient and caregiver.
- No more than 4 ounces of usable cannabis, jointly between a registered patient and caregiver.
- Do not use medical cannabis in a way that endangers the health or well-being of another person.
- Do not use medical cannabis for purposes other than medical use as permitted by law.
- Do not use medical cannabis in any moving vehicle, at any workplace, on any school grounds, or in any other public place (including public parks, beaches, & recreation centers).
- Tag all plants with your 329 Registration Number AND Expiration Date to ensure they can be easily identified as legal.
